Requesting a Remote Second Opinion

Johns Hopkins Gastroenterology & Hepatology desires to preserve the relationship you have with your local physician. As such, we seek to partner with him/her to provide the best care, locally, for you. A copy of your personalized second opinion will be sent to your physician for his/her records as well as for discussion at your next appointment.

Step 1.
Discuss with your physician why you would like to have a remote second opinion from Johns Hopkins Gastroenterology & Hepatology. Print and complete the "Consultation Request" form, and have your physician sign it. We are unable to provide this service without your physician's acknowledgement and permission.

Step 2.
Review the "Checklist" with your physician to determine which medical records are appropriate to send to Johns Hopkins Gastroenterology & Hepatology, and obtain a copy of those medical records. Understand that your physician's office and/or hospital may assess a fee for providing you a copy of your records. Johns Hopkins Gastroenterology & Hepatology is not responsible for fees charged by other health care providers in the assembling of your medical records for a remote second opinion.

Step 3.
Type the online "Patient Intake Form". When completed, print and include it with your applicable medical records.

Complete the "Medical History Questionnaire" and include it with your applicable medical records.

Type the online "Payment Authorization" form. When completed, print, sign and include it with your medical records. If payment authorization is not provided, Johns Hopkins Gastroenterology & Hepatology will be unable to provide a remote second opinion until payment has been processed via your credit card.

Please do not send correspondence via facsimile. We recommend all correspondence be sent via a courier (FedEx, UPS, DHL, registered US Postal Mail, etc.) that tracks the delivery of packages and requires a signature when the package you sent arrives at Johns Hopkins. Johns Hopkins does not assume liability for medical records lost by any courier, nor does Johns Hopkins endorse a specific courier. The patient assumes all liability related to the decision to use a particular courier or mail service.

Step 7.
Within 10 business days of receipt of medical records, a Johns Hopkins full-time faculty member will mail a written report to you and the physician that authorized your remote second opinion.
